  To apply for admission to the Holy Name School of Nursing - Registered Nurse program, please follow the instructions below. It is important that you follow the guidelines exactly to ensure your application is received and processed in a timely manner.
  • Send in the Application for Admission . It is to your advantage to fill out the application completely (please print) and return it promptly. Enclose your Application Fee of $50 Money Order Only (non-refundable).
  • Follow Instructions for Test of Essential Academic Skills (TEAS).
  • Complete the reference forms.
  • Send in official transcripts from your high school and college (if applicable.)
