Medical Care: A program that offers three levels of benefit coverage that you choose when you need medical services. Each level of benefits has applicable deductibles and copays. Prescription benefits are also available through a participating pharmacy.
Dental Care: Holy Name Medical Center offers three dental programs, Delta Dental Premier, Delta Dental Preferred, and Cigna Dental DHMO.
Vision: A program that offers an annual routine eye examination by a Holy Name affiliated Opthamologist with a copay.
Retirement Plan: A 401(k) program is available to benefit eligible employees. Employees may contribute up to $16,500 of their annual salary on a pre-tax basis. Holy Name Medical Center matches a portion of your contribution after completion of 12 months of employment. Employees are always 100% vested in their own contribution and are vested in Holy Name Medical Center contribution according to a five year vesting schedule.
Group Life Insurance: A program that offers term life coverage equal to one (1) times base annual salary (Holy Name Medical Center pays 100% of the premium cost). In addition, employees have the option to purchase additional term life coverage of up to three (3) times their annual salary and/or dependent term life coverage.
Vacation: Upon completion of three months of employment, Holy Name Medical Center offers employees annual vacation allowance based on job classification and/or years of service.
Sick Days and Personal Days: Upon completion of three months of employment, you are eligible to use accrued sick time and personal time.
Long Term Illness (LTI): Upon completion of six months of employment, accrued LTI days are available not to exceed seven days per calendar year. LTI days are available beginning on the fourth (4) consecutive day of illness.
Holidays: Upon employment, employees are eligible for the following scheduled holidays, New Year's Day, Good Friday, Memorial Day, July 4th, Labor Day, Thanksgiving Day and Christmas Day.
Flexible Spending Account: A program that offers employees the opportunity to reduce your taxable income while paying for necessary health care and/or dependent care expenses with tax-free dollars.
Tuition Reimbursement: Upon completion of three months of employment; you are eligible for tuition benefits. For eligible full time employees the maximum reimbursement is $5000 and $2500 for part time employees.
Benefit Eligibility: Full-time and part-time employees who work at least 40 hours every bi-weekly pay period or 1,000 hours per calendar year. Benefits for part time employee benefits are pro-rated based upon time worked.
Waiting Period: The first of the month following three months of continuous employment, except as otherwise noted.
