Choose from three levels of benefit coverage for medical services. Each level has applicable deductibles and copays. Prescription benefits are also available through a participating pharmacy.
Holy Name Medical Center offers three dental programs: Delta Dental Premier, Delta Dental Preferred, and Cigna Dental DHMO.
The program offers employees an annual routine eye examination by a Holy Name affiliated ophthalmologist. A copay applies.
A 401(k) program is available to benefit-eligible employees. Employees may contribute up to $17,500 of their annual salary on a pre-tax basis. Holy Name Medical Center matches a portion of your contribution after completion of 12 months of employment. Employees are always 100% vested in their own contribution and are vested in Holy Name Medical Center's contribution according to a five-year vesting schedule.
Group Life Insurance:
Holy Name Medical Center pays 100% of the premium cost for this program, which offers term life coverage for eligible employees. In addition, employees have the option to purchase additional term life coverage and/or dependent term life coverage.
Paid Time Off:
The PTO policy consists of two (2) banks of time:
- PTO Bank (Paid Time Off)
- LTI Bank (Long Term Illness)
Full- and part-time employees scheduled to work 20 or more hours per week will accrue PTO. Part-time benefit-eligible employees accrue PTO on a pro-rated basis, depending on their scheduled hours. PTO is based on position and length of service and may be taken after completion of three months of employment.
The purpose of the Long Term Illness (LTI) bank is to provide benefit-eligible employees with continuation of salary while they are out on an approved Medical Leave of Absence for their own medical condition.
Upon employment, employees are eligible for the following scheduled holidays: New Year's Day, Good Friday, Memorial Day, July 4th, Labor Day, Thanksgiving Day and Christmas Day.
Flexible Spending Account:
Employees with a flexible spending account may reduce their taxable income while paying for necessary health care and/or dependent care expenses with tax-free dollars.
Upon completion of three months of employment, employees are eligible for tuition benefits. For eligible full-time employees, the maximum reimbursement is $5,000; for part-time employees, $2,500.
