| Primary Duties |
Assists patients and their families in understanding illness and disability. Helps them resolve personal, financial and environmental difficulties, which interfere with obtaining maximum benefits from care. Assists patients and families in developing and achieving a plan of care to obtain maximum benefits. Implements activities related to maximizing efficient delivery of care and appropriate utilization of resources.
Contacts and interviews patients and families to assess and evaluate patients’
social, psychological and environmental situations. Reviews patient
information obtained from physician, staff and medical records to determine
patient needs.
Provides social work services in accordance with social work assessment and diagnosis. Conducts individual and group treatment and information sessions for patients and families with personal and environmental difficulties, which may interfere with obtaining maximum benefits from psychiatric and medical care.
Directs patients and families to appropriate community agencies and resources for financial assistance, counseling, rehabilitation, auxiliary care and long term care via written and oral communication.
Conducts, complex discharge planning for patients with serious disabilities or illnesses, assessing needs for more specialized care and arranges placement in other health care facilities, rehabilitation or extended care facilities as needed.
Participates in treatment planning meetings.
Documents social work assessment and interventions.
Maintains contact with families and agencies via telephone.
Arranges concrete services such as ambulance transportation as needed.
Maintains written and oral contact with patient and family concerning social work intervention and discharge plan.
Conducts admission and continued stay reviews as required by external review organizations, third party payers and department policy.
Identifies cases in which utilization problems are anticipated and initiates preventative measures.
Ensures the availability of externally and internally required reports and statistics.
Assists medical staff in responding to external payment denials as needed.

| Minimum Qualifications |
Education, Skills & Experience Required: MSW, must have a valid New Jersey social work license and certification, LCSW. Previous experience Psych hospital/health care setting preferred. Ability to effectively interact with patients, family members, nursing staff, physicians and outside resources. Ability to evaluate and develop realistic social work interventions on a timely basis. Ability to implement and coordinate such interventions and social work plans. Familiarity with federal and state regulation and current criteria for acute care and hospital stays. Flexibility to work long hours when needed and work well under deadline pressure. |
