What to Bring
Whether you have a doctor's appointment, or are scheduled for a procedure, having personal and insurance identification on hand will reduce stress and make your visit more pleasant. Here is a checklist of items to bring to the hospital with you.
- Photo identification, such as a drivers license, military or government issued card
- A driver's license will provide full (often legal) name, current address, date of birth, gender, a picture, and organ donor indicator.
- Birth certificate copy (often for minors)
- Passport
- Current Insurance Card(s)
- Insurance card will provide insurance company name, phone numbers, plan ID #, group number, member name, primary care physician (PCP), co-pay amounts, website URL, billing address, insured name, policy type, policy number, and other financial information. Contact the insurance company beforehand to correct any mistakes.
- Military medical card
- Medicare cards (see http://www.medicare.gov)
- Current month's Medicaid card
- Employer information for worker’s compensation claims and claim number (if available)
- Responsible party information (person responsible for the bill), this information may change due to divorce and custody issues
- Automobile insurance card (in cases of automobile accidents) and claim number (if available)
- Other demographic information
- Phone numbers, race, marital status, foreign language (interpreter needed), local or temporary address
- Advance directive or living will
- Diagnosis, reason for visit
- Attending physician - full name, address, phone number
- Pre-authorization and/or referrals
- Financial means to cover any patient-responsible balances such as co-payment or deductibles. In addition to cash or personal checks, we all accept most major credit cards such as MasterCard, Visa, Discover and American Express.
