Diabetes Management

Take Control of Your Diabetes. Taking an active role in the management of your Diabetes is an important part of living a long and healthy life. 

Diabetes can lead to numerous life-threatening complications. The key to avoiding these complications lies in good blood sugar control.  The Diabetes Center at Holy Name Hospital is dedicated to helping people living with diabetes to self manage their disease and ultimately prevent and or delay complications.

Certified by the American Diabetes Association, The Diabetes Center provides education about behavior change, prevention and care during complications.  People with diabetes are counseled by a registered nurse/certified diabetes educator on insulin self-administration, blood sugar monitoring, exercise, health, maintenance, and the importance of routine physical follow-up.  Through educational and emotional support, individuals learn how to take charge of diabetes to enable them to live a more healthful and fulfilling life.  Daytime and evening, group and one-to-one counseling sessions are offered.

The Diabetes Center Provides:

  • Comprehensive education to people with metabolic syndrome, pre-diabetes, Type 1 or Type 2 Diabetes, or gestational diabetes
  • Instructions on how to use a blood glucose meter, along with the why, and when to test, as well as keeping a blood sugar log. 
  • Education on meal planning, carbohydrate counting, food diary, diet and exercise
  • Follow up care to assess blood sugar management (A1c), and collaboration with physicians on medication adjustments.
  • Continued support to motivate/ empower patient with tools of self-care.

We also offer:

  • Group Core Curriculum Classes
  • Free community lectures
  • A monthly diabetes support group program.
