To apply for admission to the Holy Name School of Nursing - Registered Nurse program, please follow the instructions below. It is important that you follow the guidelines exactly to ensure your application is received and processed in a timely manner.
- Send in the Application for Admission which is available as a (PDF) file using Adobe Acrobat. If you do not have an Adobe Acrobat Reader, download a free copy now from Adobe Acrobat. Please PRINT. It is to your advantage to fill out the application completely and return it promptly. Enclose your Application Fee of $35 Money Order Only (non-refundable).
2. Follow Instructions for Nurse Entrance Test.
3. Complete Reference Forms.
4. Send in official transcripts from your high school and college (if applicable.)
