Holy Name Medical Center Awarded 3rd Consecutive ‘A’ in Patient Safety by The Leapfrog Group

May 2, 2025

Holy Name is pleased to announce today the medical center has earned its third consecutive “A” Hospital Safety Grade from The Leapfrog Group, an independent national nonprofit watchdog focused on patient safety. Leapfrog assigns an A, B, C, D or F grade to general hospitals nationwide based on over 30 performance measures reflecting errors, accidents, injuries and infections, as well as the systems hospitals have in place to prevent them.

“Earning a third consecutive ‘A’ grades is an important achievement for our entire team,” said Vasantha Kondamudi, MD, Executive Vice President and Chief Medical Officer at Holy Name. “We’re proud to be recognized for providing exceptional and dependable care for our communities. Since Spring 2024, our consistent ‘A’ grades from Leapfrog are a clear reflection of our unwavering commitment to safety and quality. That commitment drives our physicians, nurses, and staff to deliver excellent, personalized care to patients and their loved ones.”

The Leapfrog Hospital Safety Grade stands as the only hospital ratings program focused solely on preventable medical errors, infections and injuries that kill more than 500 patients a day in the United States. This program is peer-reviewed, fully transparent and free to the public. Grades are provided to nearly 3,000 acute-care hospitals nationwide and updated twice annually, in the fall and spring.

Earlier this year, The Leapfrog Group, in partnership with the Johns Hopkins Armstrong Institute for Patient Safety and Quality recognized Holy Name with an Award for Excellence in Reducing Nulliparous, Term, Singleton, Vertex (NTSV) Cesarean Section and Episiotomy Rates. The NTSV rate is used to assess the rate of c-sections in a low-risk patient population and is a measure of the quality of perinatal care.
